Hospice of the Chesapeake Launches Dementia Care Program

The program aims to improve patient well-being, reduce hospital visits, and alleviate caregiver burden.

Hospice of the Chesapeake has unveiled an innovative dementia care program designed to improve the quality of life for patients with dementia and their caregivers. Created in partnership with the National Partnership for Healthcare and Hospice Innovation (NPHI), the Center to Advance Palliative Care (CAPC), and Aliviado Health, the program provides free access to the "Dementia Care Patient & Caregiver Resource Guide."

This initiative supports families in managing dementia care at home, with a focus on reducing hospitalizations, managing symptoms, and empowering caregivers with essential tools and knowledge.

Dementia affects 6.7 million people in the United States, with Alzheimer's disease accounting for the majority of cases, according to the Alzheimer's Association. By 2050, the number of individuals living with dementia is expected to double, further increasing the strain on families and caregivers. Currently, an estimated 11 million Americans provide unpaid care for individuals with dementia, often without sufficient resources or guidance.

"We know that caring for dementia patients means focusing on the caregivers in their lives. Much of that support is educating the family on how to manage a disease that can be exhausting and alarming," said Dr. Sonja Richmond, Vice President of Medical Affairs & Hospice Medical Director at Hospice of the Chesapeake.

Dr. Richmond highlighted the importance of the new program, stating, "This program provides essential resources they can access within the comfort of their own homes. It can empower families to manage the challenges of dementia with dignity, compassion, and confidence. Our goal is to make sure they never feel alone on this journey."

The "Dementia Care Patient & Caregiver Resource Guide" offers best-in-class guidance for managing mid- to late-stage dementia. Developed with input from more than 100 nonprofit hospice and advanced care organizations, the guide provides practical tools and information, including:

When implemented effectively, the guide can significantly enhance the safety, comfort, and overall well-being of patients with dementia, while helping caregivers feel more supported and prepared.
